Forward copy of form to broker/investment advisor
Ensure that a copy of the completed form is forwarded to your broker/investment advisor who will be responsible
for the transfer.
Donation value
All donated securities are sold immediately upon receipt. The receipted amount is based on the unit closing price
on the day the Ontario SPCA receives the shares into our account. The net proceeds from the sale of the securities
will determine the final amount to be credited toward a project or fund.
If you wish to direct your gift to a specific Ontario SPCA Branch please indicate the name of that Branch on your
donation form.
